Hand In Hand Hospice has over 85 years of experience, with some staff adding over 10 years of dedicated services. Hand In Hand is involved in caring for the community, honoring loved ones and improved quality of life.
The Hospice team is made up of:
Physician/Nursing Care:
Physician-directed care, coordinatinated by a hospice nurse
Nursing visits for pain and symptom management with 24-hour on-call support
Training for family and caregivers
Medications for comfort
Hospice Aide:
Assistance with personal care and activities of daily living
Social Work:
Focus on patient's and family's individual situation and loss issues
Community resources
Individual and family counseling
Pastoral Care:
Indvidualized needs assessment
Spiritual support offered to patient and family
Coordination of care between the family's clergy and hospice
Volunteer:
Relief for caregiver
